  Alita Goins,RN

Ms. Goins received her B.A.  with a major in Psychology & Religion education from Hardin Simmons University in Abilene, Texas and received a B. S. in Nursing in 1987 from Texas Woman's University. She worked on Medical Surgical Units and Psychiatric Units in hospitals until August of 1999. Ms. Goins has been the school nurse at Workman Junior High for 10 years.

Clinic Services
The primary task of the clinic is to administer medications, promote student wellness and assist in emergency situations. Any student who goes to the clinic must have a pass from his/her teacher.  The clinic does not stock: contact solution - cough drops - or over-the -counter medication (such as Aspirin/Tylenol) or Ace bandages.
Only the medication which is required to enable a student to stay in school may be given at school.
A - All prescription medication must have a physician and parent/guardian signature (Get the form from the clinic).
 Over-the-counter medications must be in the original containers and will be kept and dispensed in the clinic with a parent/guardian signature.  Non-prescription or over-the-counter medication may be sent with a note from the parent with a parent signature only.
B - If it is necessary for the student to leave school due to illness, the nurse will contact the parents to make arrangements for the student to be picked up. Students should not use their cell phone to bypass this procedure.
 
 
Sign In/Sign Out

 

 #1. Students are encouraged to get doctor and dental appointments before or after school.

#2. If the school day must be interrupted, the parent must come to the receptionist's desk to sign the student in or out..

#3. If  a parent calls or sends a note early in the day, the student may be given a medical pass and will be waiting in the office at a specific time.

#4.  Students will not be releases to anyone unless their name in on the student's enrolement card with permisition from the parent.

#5. School Hours - 8:55 a.m. to 3:55 p.m. Students are not to be in the building before the 8:47 bell or after 4:10 without a pass from a teacher.

Texas State Law Requires Immunizations Against the Following Diseases.

DPT, DTaP, DT, Td, Tdap: 3 doses of any combination, including one on or after 4th birthday.  A booster is due every 10 years.  7th graders must have had a Tdap within the last 5 years.

Polio: 4 doses unless the 3rd dose was given on or after the 4th birthday.

Measles, Mumps, Rubella: 2 doses of a measles-containing vaccine.

Hepatitis B: 3 doses for children in grades Pre-K-12 in 2006-07.

Varicella (Chicken Pox): 1 dose on or before 1st birthday (or parent-validated history of chicken pox illness)
           7th Grade is now required to have a second dose beginning the 2009/2010 school year.  2 doses are also required if vaccine is given for the first time after 13 years of age or older.

Meningitis: 1 dose required for 7th graders beginning the 2009/2010 school year.  

The nurse sends out a notice at the beginning of each month if your child is due to receive an immunization that month. If your child has already received the immunization, please send a copy of the shot record to the nurse.

Screenings

Vision, Hearing, and Scoliosis Screening

All students that are new to the district will be screened in September. 
 
Next I will begin screening the 7th graders for Vision and Hearing.
 
According to policy, any child that does not pass a vision or hearing screening the first time, will then be screened a second time. At that point, I will notify parents (via a letter) that their student should be seen by a specialist.  A hearing specialist is available through the AISD and appointments may be made at no cost to the parent.  Information about setting up the appointment time will be included with the letter.  The parent must contact an eye doctor in the community to examine the student’s eyes.  However, if you do not have resources to pay for the vision appointment or glasses, please call the clinic (682-867-1206). 
 
In the spring, I will screen the 8th graders for Scoliosis.  Scoliosis is a condition of the spine that causes a abnormal curve or hump.  The State of Texas requires school nurses to examine the backs of all 8th grade students for this condition.  If the condition is recognized at this age, when the student is still growing, the condition can frequently be reversed and corrected.  Once the student has fully grown it is much more difficult if not impossible to correct.   The examination is made in privacy with only two trained nurses viewing the student's back. If an abnormality is suspected we will send a notice home to have the student’s back examined further by a physician.   I will not be screening 8th graders for vision and hearing this year. 
 
If you feel your child is having problems, please notify me and I will screen your child.
