In Accordance with rules adopted by the State Board of Education, an individual who receives special education services will by his/her sixteenth birthday have developed and annually reviewed, an individual transition plan (ITP). This is a separate document from the Individual Education Plan (IEP) and is intended to prepare an individual for community living and work after she/he is no longer receiving public school services.
