To enroll a new student,
the student must be accompanied by a parent or guardian.

| Diphtheria | 4 doses for age 6 and younger, one since age 4 and again in 10 years | |
| Tetanus/Pertussis | 3 doses for ages 7 and older, one since age 4 and again in 10 years | |
| Polio | 3 doses, one since age 4 through age 17 | |
| Measles | First dose after age 1 and second dose by age 5 for all students born after 9.2.91; OR second dose by age 12 for students born before 9.2.91 (or test of immunity) | |
| Mumps. Rubella | 1 dose of each after age 1 (or test of immunity) | |
| Hib (Haemophilus) |
3 doses by age 15 months or 1 dose age 15 months (or U.S. doctor's certificate that student has had disease) | |
| Hepatitis B | 3 doses by age 5 for students born on or after 9.2.92 3 doses by age 12 for students born between 9.2.88 and 9.1.92 |

| Varicella (Chicken Pox) |
1 dose after age 1 (or proof of illness) for students born on or after 9.2.94 1 dose (or proof of illness) by age 12 for students born between 9.2.88 and 9.1.94 |
