the student must be accompanied by a parent or guardian.

| Measles | 2 doses pf measles-containing vaccine, 1st dose on or after the 1st birthday, 2nd dose by age 5 or entering kindergarten | |
| Mumps. Rubella | 1 dose of each after age 1 (or test of immunity) | |
| Hib (Haemophilus) |
not required after age 5 | |
| Hepatitis B | 3 doses for children in grade Pre-K to 12th | |
| Varicella (Chicken Pox) |
8th - 12th grade 1 dose after the 1st birthday or if first dose is given at age 13 and older a second dose is needed. | |
