To enroll a new student, the student must be accompanied by a parent or guardian.

Immunization Information

Dtap
(Tetanus
Diptheria and Pertussis)

3 doses of tetanus-diptheris containing vaccine, with at least one dose on or after the 4th birthday. 1 dose of Td or Tdap required every 10 years there after
Polio at least 3 doses including one given on or after the 4th birthday
Measles 2 doses pf measles-containing vaccine, 1st dose on or after the 1st birthday, 2nd dose by age 5 or entering kindergarten
Mumps
Rubella		 1 dose of each after age 1 (or test of immunity)
Hib
(Haemophilus)		 not required after age 5
Hepatitis B 3 doses for children in grade Pre-K to 12th
Varicella
(Chicken Pox)		 8th - 12th grade 1 dose after the 1st birthday or if first dose is given at age 13 and older a second dose is needed.
