The date, the parent or guardian\u2019s name,\u00a0 and the service address (not the mailing address) must all be visible on the bill.<\/div>\n<div>For assistance with password resets and Parent Self-Serve questions, please contact your student\u2019s attendance clerk.<\/div>\n<div><b>If your student will NOT\u00a0be returning to Lamar High School for the 2024-2025 school year, please complete the following<\/b>\u00a0<a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLScj0SKwCKiiK2g_8df-UEYykVFOvBpjSrpFVNRIIDkQJ3IkKQ\/viewform?usp=sf_link\" target=\"_blank\" rel=\"noopener\">Google form.<\/a><\/div>\n<hr \/>\n<div><b>2024-2025 NEW STUDENT REGISTRATION<\/b><\/div>\n<div><i>Students who did NOT complete the 2023-2024 school year at an Arlington ISD campus.<\/i><\/div>\n<div>Log onto\u00a0<a id=\"OWAbaa9b74d-62b8-b09d-2ea9-c09a0849c515\" class=\"x_OWAAutoLink\" href=\"http:\/\/www.aisd.net\/\" rel=\"noopener noreferrer\" data-auth=\"NotApplicable\" data-linkindex=\"9\">www.aisd.net<\/a>\u00a0to complete the district\u2019s online registration through Parent Self-Serve and upload all required documents. The parent or guardian will then visit in-person to enroll the student and meet with their counselor to create a schedule.<\/div>\n<div>The first day of school will be on August 14, 2024.<\/div>\n<hr \/>\n<div><b>REQUIRED DOCUMENTS\u00a0<\/b><\/div>\n<ul>\n<li>Proof of Residency: Current utility bill (gas, water, or electric), mortgage statement, or lease agreement. (Must be dated within 30 days of registration.)<\/li>\n<li>Student\u2019s Birth Certificate<\/li>\n<li>Student\u2019s Social Security Card<\/li>\n<li>Photo ID of Parent\/Guardian: Driver\u2019s License, State or International ID, or Passport<\/li>\n<li>Current Immunization Record<\/li>\n<li>Report Card\/Transcript from Prior School<\/li>\n<\/ul>\n<div>If a student is living with someone other than a parent or guardian, an Educational Guardianship must be completed and notarized by\u00a0<b>both<\/b>\u00a0parties.<\/div>\n<div>If a parent or guardian and the student are living with a friend or relative, a Residency Affidavit must be completed and notarized by<b>\u00a0both\u00a0<\/b>parties.\u00a0<b><i>Residency Affidavits must be renewed yearly.<\/i><\/b><\/div>\n<div>**Students attending on a Residency Affidavit may be subject to a home visit.<\/div>\n<div><i>(Notary on campus)<\/i><\/div>\n<div><\/div>\n<div><b>REQUIRED IMMUNIZATIONS<\/b><\/div>\n<ul>\n<li>Diphtheria Tetanus\/Pertussis: At least 3 doses, with one since age 4<\/li>\n<li>Tdap\/Td booster 1 dose within last 10 years<\/li>\n<li>Polio: 3 doses, with one since age 4<\/li>\n<li>Measles, Mumps, Rubella: 2 doses<\/li>\n<li>Hepatitis A: 2 doses required K-10th<\/li>\n<li>Hepatitis B: 3 doses over no less than a 4-month period<\/li>\n<li>Varicella: 2 doses after age 1\u00a0OR\u00a0history of chicken pox illness<\/li>\n<li>Meningococcal(MCV4): 1 dose required after age 11 \/ 1 dose recommended after age 16<\/li>\n<\/ul>\n<div><b>APPLY FOR FREE AND REDUCED LUNCH<\/b><\/div>\n<div>We are committed to providing your student wholesome and nutritious food. This transfer window is first come, first served.<\/li>\n<li>It is not necessary to apply in this window if your student is currently waitlisted from a prior window.
