The parent or guardian will then bring the student to the campus to complete the enrollment process on the day the student will start school.\u00a0<\/p>\n<h4><b>REGISTRATION DOCUMENTS\u00a0<\/b><\/h4>\n<ul>\n<li aria-level=\"1\">Proof of Residency: Current utility bill (gas, water, or electric), mortgage statement, or lease agreement. (Must be dated within 30 days of registration.)<\/li>\n<li aria-level=\"1\">Student&#8217;s Birth Certificate<\/li>\n<li aria-level=\"1\">Student&#8217;s Social Security Card<\/li>\n<li aria-level=\"1\">Photo ID of Parent\/Guardian: Driver&#8217;s License, State or International ID, or Passport<\/li>\n<li aria-level=\"1\">Photo ID of the Affiant or New Guardian (If applicable)<\/li>\n<li aria-level=\"1\">Current Immunization Record<\/li>\n<li aria-level=\"1\">Report Card\/Transcript from Prior School, Current Class Schedule<\/li>\n<\/ul>\n<h4><b>EDUCATIONAL GUARDIANSHIP<\/b><\/h4>\n<p>If a student is living with someone other than a parent, an Educational Guardianship must be completed and notarized by both the parent and the guardian.<\/p>\n<ul>\n<li aria-level=\"1\"><a href=\"https:\/\/www.aisd.net\/wp-content\/files\/Educational-Guardianship-English.pdf\">Educational Guardianship Form (English)<\/a><\/li>\n<li aria-level=\"1\"><a href=\"https:\/\/www.aisd.net\/wp-content\/files\/Educational-Guardianship-Spanish.pdf\">Educational Guardianship Form (Spanish)<\/a><\/li>\n<\/ul>\n<h4><b>RESIDENCY AFFIDAVIT<\/b><\/h4>\n<p>If a parent or guardian and the student are living with a friend or relative, a Residency Affidavit must be completed and notarized by both parties. <i>Residency Affidavits must be renewed yearly.<\/i><\/p>\n<ul>\n<li aria-level=\"1\"><a href=\"https:\/\/www.aisd.net\/wp-content\/files\/Residency-Affidavit-English.pdf\">Residency Affidavit Form (English)<\/a><\/li>\n<li aria-level=\"1\"><a href=\"https:\/\/www.aisd.net\/wp-content\/files\/Residency-Affidavit-Spanish.pdf\">Residency Affidavit Form (Spanish)<\/a><\/li>\n<\/ul>\n<p>Both the Parent or Guardian and the Affiant&#8217;s name must be listed on the bottom where it says personally appeared indicating the notarization was performed for both parties.\u00a0<\/p>\n<p>**Students attending on a Residency Affidavit may be subject to a home visit.<\/p>\n<p>(Notary on campus)<\/p>\n<h4><b>REQUIRED IMMUNIZATIONS<\/b><\/h4>\n<ul>\n<li aria-level=\"1\">Diphtheria Tetanus\/Pertussis: At least 3 doses, with one since age 4\u00a0<\/li>\n<li aria-level=\"1\">Tdap\/Td booster 1 dose within last 10 years\u00a0<\/li>\n<li aria-level=\"1\">Polio: 3 doses, with one since age 4\u00a0<\/li>\n<li aria-level=\"1\">Measles, Mumps, Rubella: 2 doses\u00a0<\/li>\n<li aria-level=\"1\">Hepatitis A: 2 doses required K-10th\u00a0<\/li>\n<li aria-level=\"1\">Hepatitis B: 3 doses over no less than a 4-month period\u00a0<\/li>\n<li aria-level=\"1\">Varicella: 2 doses after age 1 OR history of chicken pox illness\u00a0<\/li>\n<li aria-level=\"1\">Meningococcal(MCV4): 1 dose required after age 11 \/ 1 dose recommended after age 16\u00a0<\/li>\n<\/ul>\n<hr \/>\n<h3><b>APPLY FOR FREE AND REDUCED LUNCH<\/b><\/h3>\n<p>We are committed to providing your student wholesome and nutritious food.
